Step 1 – Reporting the case of passport loss

Report the passport loss by ringing up the National Passport Information Center at their toll-free
number 1-877- 487-2778. Calling is easier than reporting in other ways but the problem is
getting through the line. In such situations, the second option of reporting passport loss seems
easier.
The second way of informing about lost US passport is by filling out Form DS-64, Statement
Regarding Stolen or Lost Passport. This form needs to be filled up only when the lost passport is
valid. Provide answers to the provided questions in the form in detailed manner. Once the form
fillup is complete, it has to be mailed to the address: U.S Department of State Passport Services,
Consular Lost/Stolen Passport Section, 1111, 19 th Street, NW, Suite 500, Washington DC 20036.
Important notes in regard to stolen or lost passport
Here are few notes that have to be kept in mind regarding stolen or lost passport:
Passports which have been reported stolen or lost either by filling up Form DS-64 or by
telephone are considered invalid and cannot be used for travel.
If the passport is recovered after reporting loss or stolen, submit the same to the physical
address of US Department of State. On submission and on request of the candidate, the
passport will be returned after cancellation. Otherwise it is destroyed.

Once the passport is reported to be stolen or lost, there is no revalidation.
Step 2 – Application for new US passport – Filling up Form DS-11
Generally when people apply for renewal of their passport via mail, the undamaged and previously
issued passport needs to be furnished with the same as well. However, for people who are applying for
lost passport renewal, this criterion will not be valid. This is the reason they will need to fill out Form
DS-11 and apply for the new passport. There is no separate lost passport form; application form for a
new passport is all that is needed.
There are two ways in which Form DS-11 can be completed. The form can be filled up online and then a
print out of the same can be taken. Or printout of the empty form can be taken and has to be filled up
using black ink pen. Make sure that the form is filled out legibly and in error-free manner. Do not put
signature on the application form until and unless the acceptance agent asks to do so.
Step 3 – Collecting all supporting documents
For replacing the passport for a lost or stolen case, all documents that are needed for a fresh passport
are required. The documents will accompany the application form. Important documents that will be
needed are:
US citizenship proof
Identity proof
One copy of the identification proof document
1 passport size photo (recent)
The document which is furnished as proof of US citizenship should be in original or should be a certified
copy. After the application is successfully processed, this document is returned to the applicant.
If the case of passport lost or stolen is for a minor, guardians or parents applying for a passport
replacement have to provide evidence for establishing relationship with the child. Photo identifications
of both parents and guardians are needed. Single parent or guardian can also apply provided evidence
of sole legal custody of the child can be furnished.
Visiting passport acceptance facility
Duly filled up and completed Form DS-11 along with the supporting documents have to be submitted to
an authorized acceptance agent at the passport acceptance facility. These offices are often in post-
offices, municipal government offices, counties or in public libraries. Appointments might be needed in
some facilities.
On submission, an acceptance agent checks through the application form and the supporting documents
thoroughly. After an oath, the application form needs to be signed before the agent. Passport
application fee payment needs to be made in favor of the US Department of State; while the execution
fee has to be paid to the acceptance agent.
For standard processing, estimated time taken is about 6 weeks. However, expedite service can be
