Step 1 - Filling up of Form DS-11
Like adults, Form DS-11 needs to be filled up for applying for child passport too. There form can be filled
up online as well as offline – as per the convenience of the applicant.
For filling up the form offline, download the empty form and get it printed. Use black ink pen only for
filling up the form legibly. Illegible handwriting can lead to cancellation of the form or lead to errors in
the passport. Do not sign the form unless the acceptance agent asks to.
Form DS-11 can also be filled up online. Once the fill up is completed, print the form.
The main form covers 2 pages with approximately 22 questions. You need to provide detailed personal
information like full name, address, contact details, sex, age, place of birth, emergency contacts etc.
Step 2 - Gather all the needed supporting documents to be attached with the form
This is the most important part of processing a minor passport in US. There are different kinds of
documents that need to be submitted with the application form for getting a child passport in the
country. The important documents include:
Evidence of US citizenship of the child
Photo ID of the child or of parents/guardians
Guardians’ or parents’ relationship proof with the child
Photocopies of identification documents
One passport photo of the minor measuring 2x2 inches and taken within last 6 months
Documents which work as proof towards parental relationship
U.S birth certificate
Foreign land birth certificate
Consular Report of Birth Abroad of US citizen (FS-240)
Guardian(s) or parent(s), applying for the passport of the kid must furnish their photo ID proof if the
child already does not have one. The photo ID proof of the parent or the guardian can be valid driver’s
license or valid and undamaged passport. Photocopies of the documents also need to be submitted. The
copies should be made on plain white paper measuring 8.5 x 11 inches. Images will be on one side of the

One guardian/parent is not able to appear
Duly signed and notarized Form DS-3053: Statement of Consent from non-applying guardian/parent
needs to be submitted along with Form DS-11.
One guardian/parent is absent and cannot be traced
The parent who is applying for passport for children has to submit Form DS-5525: Statement of
Exigent/Special Family Circumstances. There should be detailed explanation of the non-applying
guardian’s/parent’s unavailability. The statement should also mention recent efforts made for
contacting the absent parent/guardian. The applying parent might need to furnish evidence for
documenting his/her claim of special circumstances or of exigent. If the statement seems insufficient,
the passport agency might ask for other additional details from the applying parent/guardian.
When the minor has single parent/guardian
Proof has to be submitted regarding sole authority for applying passport for the child. The documents
that are considered in this regard include:
Court order granting sole legal custody to the parent applying for the child passport
Consular Report of Birth Abroad, Foreign or US birth certificate and adoption decree, where the
applying parent is only listed as the sole legal parent/guardian of the child
Judicial declaration regarding incompetence of the non-applying parent/guardian
Court order permitting specifically the applying parent’s/guardian’s travel with the kid/minor
Other parent’s/guardian’s death certificate
When both guardians/parents are not available
Though this case is rare, but it might happen and hence there is a clause with the US State Department
for the same. A third party is allowed to apply for the child passport, but with some documents in place.
Both parents/guardians have to provide an affidavit or a statement which will be duly signed and
notarized. The statement will provide authorization to a third party for applying on behalf of the child.
The parents’ or guardians’ photocopied identification documents have to be attached to the affidavit or
the statement. If the statement comes from one parent only, sole custody of the parent has to be
established through valid documents.
Step 3 - Visiting the application acceptance facility
This is the last step as how to get a child passport in USA. After the form is filled up and all necessary
documents collected, visit the nearest passport application acceptance facility. Along with submitting
the form and the necessary documents, child passport fee also needs to be paid. The check for the
application fee will be paid to US State Department. An execution fee also needs to be provided to the
attending agent at the acceptance facility.
