Gather Required Documents
Before starting the application, gather all necessary documents. You will need proof of U.S. citizenship for the minor, which can be a U.S. birth certificate, Consular Report of Birth Abroad, Certificate of Citizenship, Naturalization Certificate, or a valid (even expired) U.S. passport.
Proof of parental relationship is also required. This can be established with a U.S. birth certificate, Consular Report of Birth Abroad, adoption decree, divorce or custody decree, or a court order establishing guardianship.
Both parents or guardians must provide valid identification, such as a driver’s license, foreign passport, Certificate of Naturalization, or a valid U.S. passport.
Parental Consent
Both parents or guardians must appear in person with the child. If one parent or guardian cannot appear, they must provide a notarized Form DS-3053 (Statement of Consent).
Complete the Application Form
The application form for a U.S. passport is Form DS-11. You can fill it out online, print it, and complete it by hand. The form is available on the State Department’s website.
Provide a Passport Photo
You will need a passport photo of the minor. The photo must be in color, taken within the last six months, and 2 x 2 inches in size. It should have a plain white or off-white background, and the head must be 1 to 1 3/8 inches from the bottom of the chin to the top of the head.
Pay the Fees
The fees for a passport for minors under 16 are $100 for a passport book, $15 for a passport card, and $115 for both. There is also a $35 execution (acceptance) fee.
In-Person Appointment

Both parents or guardians and the child must appear in person at the appointment. This is a crucial step as it allows the officials to verify the identity of both the parents/guardians and the child.
