1. First-Time Passport Applicants
For first-time applicants, you’ll use form DS-11, which can be found on the State Department’s website. Print and complete the form by hand (black ink only), or type the application using the online form filler.
You will need to make an in-person appointment at a passport acceptance facility. This is because first-time passport applicants must have their identity verified by an agent. The DS-11 must be completed but not signed until an agent verifies your identity. Make sure you bring the original documents or certified copies when applying.
You will also need these documents for a New Passport:
Proof of Citizenship
U.S. birth certificate, naturalization certificate, or Certificate of Citizenship.
Proof of Identity
Driver’s license or government-issued ID.
Passport Photo
One recent passport-sized photo (2×2 inches). Make sure your photo meets the Department of State’s requirements.
2. Passport Renewals
If you are renewing your passport, Complete the DS-82 form. Print and complete the form by hand (using black ink only), or type the application using the online form filler. Make sure to sign the form before submitting it.
To renew your passport by mail, you’ll have to print the form and mail it with your application. There is also the option for online passport renewal, but it has different eligibility requirements.

Parental Proof of Identity
